    Payment & Insurance
    In order to offer the best quality of care, Dr. Martínez is an out-of-network provider. This means that the patient/parents and Dr. Martinez are collaborative decision makers regarding care – not the insurance companies or agency administrators. All treatment is  personalized and you are not locked into a diagnosis.

    Sessions are paid for out-of-pocket. Helping patients afford treatment, however, is important. Many insurance plans offer out-of-network benefits and, if needed, Dr. Martinez can provide a itemized receipt (or a “superbill”) for the services provided, which you can submit to your insurance company. It is your responsibility to contact your insurance company to determine out-of-network benefits.

    If you would like to utilize your out-of-network insurance benefits, please confirm your benefits directly with your insurance provider and ensure that the visits are covered. It may be helpful to provide the billing codes that may be utilized for your treatments:

    • Initial Evaluation: 90791
    • Psychotherapy only: 90834 (45 minutes) or 90837 (60 minutes)
    • Family/Couple’s therapy: 90846 (without pt.) or 90847 (with pt.)

    Fees are due upon completion of the visit. Payment can be made via cash, check, IVY pay (for HSA cards), Zelle, or Venmo.

    Late Cancellations and No-Shows
    Since the scheduling of an appointment involves the reservation of time set aside especially for you, a minimum of 48 hours’ notice or 2 business days (whichever is longer) is required for rescheduling or canceling an appointment. If an appointment is cancelled with less than 48-hours’ or 2 business days’ notice, the full fee will be charged. Please note that your insurance will not cover the cancellation fee.
